This workshop series assists self advocates and their families in exploring the options available when they complete their education. This series provides the information needed to begin the process of Person Centered Planning. Individuals who complete the program may be eligible to apply for funding to initiate their plan, as well as be ready to create and submit a plan to the local Developmental Disabilities Service Office to request additional support.

This workshop series is a collaborative effort between The Advocacy Center, the Western Regional Transition Coordination Site and The Self Advocacy Association of New York State.

To register for this program you must have a developmental disability, live in Monroe County and live with your family. In order to be eligible for financial support, participants must complete all five sessions.
