CQC Seeks Input on Quality of Life

PDF CQC quality brochure and questionnaire (PDF 123.36 Kb)

The New York State Commission on Quality of Care and Advocacy for Persons with Disabilities, in partnership with The Coalition to Create Quality, is seeking input.

Their brochure states, "We want to learn from people with disabilities and their families about their quality of life. Do you think your life is meaningful? Are you happy? What are the challenges faced by people with disabilities to have a good quality of life?

"The answers to these questions will help make services and supports more responsive to people with disabilities.

"If you would like to be part of the movement to help create a good quality of life for people with disabilities, please consider responding to our Quality Questionnaire. We will not share any information without your permission."

See the attached PDF for a copy of the brochure and questionnaire.
